A complete description of the proposed workshop.
Please write in complete sentences. Include: medium, title, objective of workshop/student goals, and process.
Appropriate age range for workshop participants.
(Artist workshops are often held in schools. Please specify if workshop is designed for elementary-, middle-, or high-school students, or if workshop is strictly for adults.)
Description, quantity, and estimated cost of equipment/supplies required.
Please figure quantities based on a maximum of 15 participants for one five-hour workshop. You must work within a budget of $100 for a five-hour workshop. Because of budget constraints, proposals for workshops with supplies that cost more than this amount cannot be considered. Please indicate whether an item listed can be reused for multiple workshops. Specify brand names, if possible.
Description of community lecture/demonstration or performance (maximum one hour). Include title and technical requirements.
The names, addresses, and telephone numbers of three references.

PLEASE ATTACH:
A current resume. (Indicate with an asterisk what you consider to be your five most significant achievements.)
CD-Rom containing six to ten images in JPEG format, or one film or video tape (1/2” VHS or ¾” format) of your recent work executed in the medium of the workshop proposal.
Do not submit original work or samples. All performing artists must submit a video tape and/or CDs; mark video with title of work(s) and indicate exact counter location of each.
Visual artists include image list with medium, date, and dimensions.
Self-addressed envelope with adequate postage for the return of slides, videos, or films.
Self-addressed, stamped postcard marked “Entry received” to confirm receipt of application (optional).
